Impacted Teeth: Orthodontic and Surgical Considerations

Most permanent teeth erupt into the mouth.  In some individuals, however, the permanent teeth may fail to erupt and become impacted within the bone and/or soft tissue.  When this lack of eruption occurs, two alternative treatment plans are possible.  The impacted tooth could be extracted, or could be surgically uncovered and moved into the dental arch orthodontically.  A comprehensive examination procedure is completed to determine the predictability of exposing the impacted tooth.  This examination may require a CT scan to obtain the necessary information to recommend a predtable treatment plan.

Impacted teeth can be classified as simple or complex.  Simple impacted teeth are located close to the surface and usually require less surgical procedures and treatment time to bring the tooth into the mouth.  Complex impacted teeth can require multiple surgical procedures and an extended treatment time. IV conscious sedation is generally given during the exposure of complex impacted teeth. Dr. Chadwick has worked with numerous orthodontists to successfully uncover impacted teeth.  His preference is to have the braces placed and room created for the impacted tooth prior to the surgical procedure.

Surgery is completed in the office using sonscious sedation, when needed, and local anesthesia to insure a comfortable patient.  Bracket and chain devices are bonded to the impacted tooth crown to aid the orthodontist to guide the tooth into the dental arch.  Treatment time can be many months to over a year.  Progress is continually monitored in the orthodontist’s office.  Dr. Chadwick will consult with the orthodontist periodically as needed.
